• Membership in health clubs and weight loss clinics & herbal medical centers
  
• Care and treatment of enhanced fertility using drugs and artificial methods
  
• Drugs not prescribed by physician within out network
  
• Major Dental surgery & Major Ophthalmologic surgery and cosmetic/plastic surgery
  
• Cancer care: Oncology test, Drugs, Chemotherapy and Radiotherapy
  
• High Cost surgical procedure, including organ transplants, e.g. open heart surgery & neurosurgery
  
• Specific treatment for HIV/AIDS i.e. Anti-Retroviral Drugs
  
• Prolonged Intensive care treatment for myocardial infraction, severe burns and major complex surgeries
